Community Fund Grant Application

Please use this form to apply for Community Fund Grants.

Before applying, please read this year’s Community Fund Grant Guidelines.

Please note that page 5 of the guidelines lists application requirements, which vary depending on the grant amount requested, and which you will need to know in order to have the correct information at hand when filling out this form.

The form does not allow you to save your work and return to the form later. Once you click “Submit,” all information will be submitted. Please prepare your answers and files separately and have them all ready when you begin this form.

  • Please enter just the amount you are requesting for one year. Applicants invited to submit multiyear grants do not need to submit multiyear budget amounts.
