Please note that page 5 of the guidelines lists application requirements, which vary depending on the grant amount requested, and which you will need to know in order to have the correct information at hand when filling out this form.

Only required for requests of $2001 and above. Please ensure the permit is currently valid. Organizations not subject to the Utah Charitable Solicitations Act may submit their state waiver from the Act in lieu of a permit.
