Utah’s Zero To Eight Care Coordination Program Helps Connect Families With Valuable Resources for Their Kids

Ever wonder where to turn when your child needs some type of service or assistance? Utah’s Department of Health and Human Services has care coordinators available to help parents and caregivers navigate the childhood system in Utah. These coordinators serve as a centralized access point to help link children and their families to internal, external, and community-based services and programs that serve children. For example, they can help with: 

  • Identifying any developmental delays and connecting families with related services 
  • Home visiting programs to support parents  
  • Basic needs (food, formula, diapers, etc.) 
  • Lactation support 
  • Maternal mental health 
  • Free educational resources 

They will make the appropriate referral appointments while working with the families, will report back to any referring agency/provider to provide a closed loop referral system, and will do follow-ups with families.  

This free, statewide service does not have any eligibility requirements and ensures families receive the care and support they need in their native language, either through a Spanish-speaking care coordinator or using an interpreter service.
