Apply for Youth Mental Health and Substance Abuse Prevention Grants

Grants Purpose

In keeping with the mission of the Mental Wellness Alliance, Park City Community Foundation is seeking Letters of Intent (LOI) from organizations working to improve mental health outcomes and reduce substance use among youth (18 and under) in Summit County, UT, with an emphasis on the middle and high school aged population.

These funds must be used to further one or more of the following strategic priorities:

  1. Offer meaningful programming in support of youth mental health outcomes; and/or
  2. Provide data-driven initiatives that reduce youth substance use; and/or
  3. Increase protective factors that reduce youth substance use.

Approaches should align with the Seven Strategies for Creating Effective Community Change. Learn more about risk and protective factors for substance use among youth here and impact on the healthy development of children and adolescents here.

Areas of Support

Mental health and human services, youth services, and other efforts.

Types of Support

Existing or new programs/projects; equipment; consulting services; matching funds; technical assistance; start-up funds; and joint requests from multiple applicants.

Grants Timeline

July 17: Application period opens.

August 4: Deadline to submit an online Letter of Intent.

September 8: Deadline to submit online application (by invitation only).

October 13: Applicants notified of Steering Committee decision.

Grant Amounts

Grants will fall in the range of $10,000-$50,000. The Steering Committee does have authority to award grants outside of this range.

Steering Committee

The Youth Mental Health Steering Committee is composed of nine community members, at least one of which is a Community Foundation Board Member.

Staff

Community Foundation staff process and prepare grant applications for review by the Steering Committee. They serve and assist the Steering Committee, but do not advise on grant award decisions. After grants are awarded, staff and committee members will participate in the grantee reporting process (see reporting requirements below).

Eligibility 

Park City Community Foundation will consider grant applications from organizations meeting all the following eligibility requirements:

  • Serve people living or working in Summit County (Wasatch County will be considered, although grant making decisions will emphasize organizations based in Summit County that serve Summit County).
  • Qualify as a 501(c)(3) nonprofit organization under the Internal Revenue Code.
  • Applicants without 501(c)(3) status, but which have applied to the IRS (Internal Revenue Service) for such status, may apply. Receipt letter from the IRS of application is required at time of application to Park City Community Foundation.
  • Applicants without 501(c)(3) status, but which are operating under an organization qualified as a 501(c)(3) organization, may apply separately if they have the written consent of the qualified organization. In that case, the application must contain a letter of agreement between the two organizations, which sets forth the responsibilities of each organization.
  • Applicants that are not 501(c)(3) organizations but are implementing charitable activities for the benefit of the citizens of Summit County, should contact Park City Community Foundation before applying. Under some circumstances, they are eligible to be considered for funding but may be required to adhere to separate reporting measures and disbursement processes.

Only one application per organization or alliance per grant cycle will be considered. An alliance may be a collaborative effort between two or more nonprofit organizations. The alliance would then submit one application.

Restrictions

Grants will not be awarded for:
debt reductions or retiring past operating deficits
sponsorships or one-time events
fellowships or other grants to individuals
loans
litigation
political campaigns
endowment funds
graduate and post-graduate research
purposes that further political or religious doctrine

Community Foundation’s Grantmaking Due Diligence Policy

The Community Foundation will not make grants to organizations known to support, or who engage in activities, or who publish language that incites or engages in violence, intimidation, harassment, threats, or defamation targeting an individual or group based on their actual or perceived race, color, religion, national origin, ethnicity, immigration status, gender, gender identity, sexual orientation, or physical or mental ability. These activities are contrary to Community Foundation’s mission and its commitment to diversity, equity, and inclusion.

Applications

Applications will be submitted in two phases:

  1. Letter of Intent
    1. Request Name
    2. Request Amount
    3. Request Description: Please provide 1-2 paragraphs explaining how you plan to use the requested funding. Please indicate the counties the project will serve.
    4. Additional Materials – Optional
  1. Grant Application (by invitation only)
    • Approach – max. 10,000 characters (with spaces)
      • Background: Applicants must provide a description of the nature and scope of the problem in the community, including the risk factors and local conditions that are driving youth substance use and/or poor mental health. Include data to demonstrate the problem.
      • Purpose: Applicants must describe in 2-3 sentences specifically how their application will address the public health problem as described above.
      • Outcomes: Applicants must clearly identify the outcomes they expect to achieve by the end of the project period. Outcomes are the results that the program intends to achieve and usually indicate the intended direction of change (e.g., increase, decrease).
      • Strategy and Activities: Applicants must provide a clear and concise description of the strategies and activities they will use to achieve performance outcomes. Applicants must select existing evidence­ based strategies that meet their needs or describe how these strategies will be evaluated over the course of the project period.
      • Collaborations: Applicants must describe how they will collaborate with programs and organizations either internal or external.
      • Target Populations and Health Disparities: Applicants must describe the specific target population(s) in their jurisdiction and explain how such a target will achieve the goals of the award and/or alleviate health disparities. The applicants must also address how they will include specific populations that can benefit from the program.
    • Applicant Evaluation and Performance Measurement Plan – max. 5,000 characters (with spaces)
      • Applicants must describe how they will collect data and use evaluation findings for continuous program quality improvement.
      • Applicants must describe how they will monitor and evaluate the success of the goals and objectives of their strategy and activities.
    • Organizational Capacity or Applicants to Implement the Approach – max. 5,000 characters (with spaces)
      • Applicants must demonstrate the capacity of the organization to manage the programmatic and fiscal requirements of the grant.
      • Applicants must identify the key personnel and their roles and responsibilities.

      • Organizational Budget (current).
      • Project Budget and Approach. Please upload a detailed project budget; option to include a short description of how funds will be utilized.
      • Utah Solicitation Permit. Please ensure the permit is valid. Organizations not subject to the Utah Charitable Solicitations Act may submit either:
        1. Their state waiver from the Act in lieu of a permit, or
        2. A statement on letterhead as follows: [Organization Name] is exempt from needing to obtain a Utah Charitable Solicitation Permit, per Section 13-22-8 “Exemptions” of the Utah Charitable Solicitations Act.
      • Most recently submitted IRS Form 990.
      • Other File. If ​you have other documents you wish to share, please combine them into a single ​pdf.

Review and Selection Process

All applications are screened by staff for Eligibility, Restrictions, and Grantmaking Due Diligence Policy. Applications will be reviewed in two phases:

  1. LOI Review: Letters of Intent will be reviewed for strategic alignment and completeness by the Steering Committee. The committee reserves the right to use its own best judgment when inviting applicants to advance. Applicants will be notified that their applications did not align or meet requirements.
  1. Steering Committee Review: The Steering Committee will use the following 3 criteria areas to review applications. These criteria are not exclusive, and the committee reserves the right to use its own best judgment when making final grant awards.

    2. Evaluation and Performance Measurement
    3. Applicant’s Organizational Capacity to Implement the Approach

Approach Maximum Points: 60

In this section, points are awarded based on the extent to which the applicant addresses:

  • Background: Defines the nature and scope of the problem in the community, including the risk factors and local conditions that are driving youth substance use and/or poor mental health. Applicants should include data to demonstrate the problem. (10 points)
  • Purpose: Summarizes how their application addresses youth substance use and/or mental health outcomes in their communities, including the mission of the organization as it relates to youth mental health outcomes and/or prevention of youth substance use. (10 points)
  • Outcomes: Aligns longer-term goals with the activities and strategies outlined in the Seven Strategies for Creating Effective Community Change or describes how these strategies will be evaluated over the course of the project period. (10 points)
  • Strategy and Activities: Proposes a plan that provides a comprehensive approach to support youth mental health outcomes and/or prevent youth substance use using the Seven Strategies for Creating Effective Community Change or describes how these strategies will be evaluated over the course of the project period. (10 points)
  • Collaborations: Describes the collaboration with other organizations, leaders, and key partners who have a role in conducting the proposed activities and achieving the project goals. (10 points)
  • Target Populations and Health Disparities: Defines the population of focus and rationale for selection. Addresses health disparities and promotes health equity, including populations at risk and geographic areas served. (10 Points)

Evaluation and Performance Measurement Maximum Points: 20

In this section, points are awarded based on the extent the applicant:

  • Describes how the organization will collect data for youth mental health outcomes and/or prevention of youth substance use. (10 points)
  • Describes how the organization will monitor and evaluate the success of the goals and objectives of the Strategy and Activities plan. (10 points)

Applicant’s Organizational Capacity to Implement the Approach Maximum Points: 20

In this section, points are awarded based on the extent to which the applicant:

  • Demonstrates the capacity of the organization to manage the programmatic and fiscal requirements of the grant. (10 Points)
  • Identifies the key personnel and their roles and responsibilities. (10 Points)

Select information from applications may be shared. Any organization not wishing to have their information shared should contact minda@parkcitycf.org to request that their information not be shared.

Reporting Requirements

Grantees participate in an in-person grantee forum with staff and committee members and present a short grantee report in Autumn 2024. This is a robust and engaging way to share your successes, lessons learned and vision with the Community Foundation and fellow grantees.

Publicity Guidelines

As a recipient of a grant from Park City Community Foundation, it is important to the Community Foundation, our donors, and the community that the grantee publicize and recognize the philanthropic support that a grantee receives. Park City Community Foundation requires the following of grant recipients:

  1. Grantee agrees that a notice will be included in all announcements, promotional and other appropriate material stating: “This [activity/event/organization] is supported by a grant from Park City Community Foundation.”
  2. Grantee agrees to include Park City Community Foundation’s Community Fund logo in all brochures, notices, signs, and plaques that recognize funders at events or on facilities. Please contact staff for a copy of the logo.
